Please complete the emailed paperwork and provide the requested documents so we can create your health profile. You will also need to keep a card on file for self-pay charges or copays and provide a copy of your photo ID or driver’s license.

Please allow 1–2 business days for us to create your profile after your paperwork and documents are received. Once that is complete, we can schedule your first appointment.

Your first appointment lasts up to one hour and begins with a casual meet and greet. Your provider will review your overall health and mental well-being, discuss your lifestyle using the 7 Pillars of Health model, and complete a thorough psychiatric evaluation. Together, you will review options and set realistic goals for care.

In some cases, laboratory data or additional testing may be needed for accurate diagnosis and symptom monitoring. A follow-up appointment, typically up to 30 minutes, is usually scheduled a few weeks later to review results, monitor symptoms, and adjust medications if needed.
